Liability Waiver

LIABILITY WAIVER: I am aware that participation in the Golden Standard Athletics has some inherent risks and injury can occur. On rare occasions these injuries can be serious. In consideration of my child being allowed to participate in the Desert Heat Basketball League, I, the parent or legal guardian, assume the risk of all injury and agree not to sue the Golden Standard Athletics, the directors, coaches, assistant coaches, agents, or volunteers for any and all injuries caused by or resulting from participating in the Golden Standard Athletics.

Media Release

I, hereby grant permission to Golden Standard Athletics and its affiliates to use my child’s likeness, including but not limited to photographs, video recordings, and audio recordings, for promotional and marketing purposes related to the Golden Standard Athletics. I understand and agree that: The images captured may be used in various promotional materials, including but not limited to websites, social media platforms, brochures, advertisements, and other promotional materials. I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any claims to compensation for the use of my likeness. I release Golden Standard Athletics and its affiliates, as well as their respective employees, contractors, and volunteers, from any liability for any claims by me or any third party in connection with my participation in the Golden Standard Athletics or the use of my likeness as described herein. This release shall be binding upon me and my heirs, legal representatives, and assigns. I acknowledge that I have read and understand this release and voluntarily agree to its terms.
