Summer Skills Lab
Participant Waiver.

The Parent/Guardian acknowledges that participation in basketball, athletic drills, and physical activities involves inherent risks of injury, including but not limited to: sprains, strains, and muscle pulls; fractures, dislocations, and bone injuries; head, neck, and concussion injuries; heat-related illness, dehydration, and exhaustion; cuts, bruises, and abrasions from contact with other athletes or equipment; and any other injury that may occur during athletic participation.

The Parent/Guardian voluntarily acknowledges these risks and agrees that the athlete's participation in the GSA Summer Skills Lab is entirely voluntary. The Parent/Guardian accepts full responsibility for all risks associated with the athlete's participation.

In consideration of the athlete's participation in the GSA Summer Skills Lab, the Parent/Guardian, on behalf of themselves and the athlete, hereby RELEASES, WAIVES, DISCHARGES, and COVENANTS NOT TO SUE Golden Standard Athletics, its founder, coaches, staff, volunteers, and affiliated organizations (collectively "GSA") from any and all liability, claims, demands, losses, damages, and causes of action of any kind or nature, whether known or unknown, arising out of or in connection with the athlete's participation in the Summer Skills Lab, including any claims resulting from the negligence of GSA or its staff.

This release includes, but is not limited to, claims for personal injury, property damage, wrongful death, or any other loss that may occur before, during, or after participation in the Summer Skills Lab program or any related activities.

The Parent/Guardian certifies that the athlete is in good physical health and has no known medical conditions that would prevent safe participation in physical activity, and has disclosed all relevant medical conditions in Section 1.

In the event of a medical emergency during any GSA activity where the Parent/Guardian cannot be reached, the Parent/Guardian authorizes GSA staff to: call 911 or emergency medical services; consent to emergency medical treatment on behalf of the athlete; and transport the athlete to the nearest emergency medical facility. The Parent/Guardian agrees to assume full financial responsibility for any medical expenses incurred.

By signing this waiver, the Parent/Guardian agrees that the athlete and all accompanying family members will: treat all coaches, staff, athletes, and participants with respect at all times; follow all instructions given by GSA coaches and staff; arrive on time and be prepared to participate fully; refrain from bullying, harassment, or disrespectful conduct; and respect Countryside Rec Center and leave the area clean.

GSA reserves the right to remove any athlete or family member from the program at any time for conduct that conflicts with the Golden Standard of Discipline, Confidence, Teamwork, and Excellence. No compensation will be provided for removal due to conduct violations.

The Parent/Guardian grants Golden Standard Athletics the irrevocable right and permission to photograph, video record, and capture the likeness of the athlete during any GSA Summer Skills Lab session, event, or related activity.

The Parent/Guardian gives full permission for GSA to use the athlete's photos, videos, and all likeness — in whole or in part, altered or unaltered — in any and all media, including but not limited to: Instagram, Facebook, TikTok, YouTube, and all social media platforms; the GSA website and email communications; promotional flyers, banners, and printed materials; press releases, news features, and community publications.

All photographs, videos, and media content featuring the athlete captured by or on behalf of Golden Standard Athletics are and shall remain the sole and exclusive property of Golden Standard Athletics. The Parent/Guardian waives any right to compensation, review, approval, or further consent for any such use. GSA may use this content in perpetuity without additional permission.

The Parent/Guardian agrees to indemnify, defend, and hold harmless Golden Standard Athletics, its founder, coaches, staff, and volunteers from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorney's fees) arising out of or in connection with the athlete's participation in the Summer Skills Lab, including any claims arising from the Parent/Guardian's own negligence or the athlete's conduct.

This waiver shall be governed by the laws of the State of Arizona. Any disputes shall be resolved in Maricopa County, Arizona. If any provision of this waiver is found to be unenforceable, the remaining provisions shall continue in full force and effect.
