All required waivers and participant agreements for Golden Standard Athletics programs. Please complete the appropriate waiver before your athlete's first session.

Participation waiver for Desert Heat, Desert Wolves, Desert Sky, and Desert Elite travel team members. Required before first practice.
Participation waiver for Little Legends multi-sport program. Soccer, Basketball, Tee Ball & Flag Football. Required before first session.
Participation waiver for the GSA Flag Football program. Available when registration opens.
Participation waiver for the GSA Volleyball program. Available when registration opens.
