All required waivers and participant agreements for Golden Standard Athletics programs. Please complete the appropriate waiver before your athlete's first session.

Required for all athletes participating in the free Sunday morning basketball training sessions. Discovery Group (Ages 5–9) and Skills Group (Ages 10–14).

Participation waiver for Desert Heat, Desert Wolves, Desert Sky, and Desert Elite travel team members. Required before first practice.
Participation waiver for the free Sunday volleyball strength & conditioning program. June 7 – July 19, 2026.
Participation waiver for the GSA Community Day event at Surprise Community Park. Free for all ages and families.
